Advanced DirectivesIf you want to inform your provider about your wishes regarding medical treatment during a time when you are physically unable to explain those wishes, then you should consider an Advanced Directive. Advanced directives fulfill several objectives, including: | | » | Stating your choice about the medical treatment you receive; and | | | » | Naming another person to make decisions for you if you become unable to make those decisions yourself. |
If you have questions, please talk to your Health Care Provider or contact the Outreach Department.
