Advanced Directives

If you want to inform your provider about your wishes regarding medical treatment during a time when you are physically unable to explain those wishes, then you should consider an Advanced Directive. Advanced directives fulfill several objectives, including:

»  Stating your choice about the medical treatment you receive; and
» Naming another person to make decisions for you if you become unable to make those decisions yourself.

If you have questions, please talk to your Health Care Provider or contact the Outreach Department.
